What is a Geriatric Care Manager


A Geriatric Care Manager is a professional who specializes in helping older people and their families with healthcare issues and long-term care arrangements. As people live longer and families are separated by many miles, there is an increased need for care managers who can bridge the distance between aging seniors and their loved ones to assist with day-to-day living and long-term care.

As mom, dad or another relative or close family friend ages, it may be challenging to determine the best course of care. How can he or she live independently? When is it time to look for a new living situation? What is the level of care currently needed, and what do we do when this changes? Often family members search for the answers within themselves, unaware that there are experienced and compassionate geriatric experts available for support and direction.

  • provide strategies for your parent to stay safe and independent at home
  • coordinate all of your parent’s care
