The Parent Care Process


How Does it Work…

1. Meeting

We meet with your parent to assess what’s going on in their day-to-day lives. In a casual, non-threatening way, we evaluate the current medical situation, physical needs, psychological and social functioning, environmental and safety needs of your parent. This gives us the information we need to develop a plan of care.

Some families prefer to be involved in the assessment interview, and some prefer not to be involved—it’s entirely up to you. During the assessment, our focus is on your parent so we can gain a better understanding of their level of functioning.

2. Recommendations

After the assessment, we prepare a summary of our findings, with a list of recommendations for your parent’s ongoing care. We’ll go over this in detail with you, and together we will formulate a plan of care. We encourage family involvement in the planning stage, if possible, so everyone understands the results of the assessment and our recommendations.

3. Implementation

Based on our agreed upon plan, Parent Care begins implementation. We coordinate and manage all of the services your parent needs to ensure a safe and productive living environment. We accompany your parent to doctor appointments to be an advocate, and prepare a written summary of each visit so you stay informed. If needed, we hire nursing staff, coordinate medication management, secure personal and support services, etc.

We are available 24 hours a day, 7 days a week, and available in the event of a crisis so your parent will not be alone.

Our one-on-one visits are a time for observation to monitor and check on your parent to be sure everything continues to work as planned.

If your parent needs a new living situation, we determine the appropriate level of care needed, then screen and recommend suitable living environments in the area. We are aware of the options available in the area, and will assist with the application, placement process, and life-changing move. During, and especially after the move, we oversee your parent’s care to provide the care and support they need during this transition.

4. Family Outreach

As the eyes and ears for our client’s family, we will provide monthly updates to keep you aware of your parent’s situation. Your care manager will write, e-mail or call depending on your preference.

Your care manager is also available to act as a liaison to help families and older adults with difficult life decisions and issues. Differing opinions about what is best for a parent can sometimes cause conflicts among family members, and your care manager is an objective professional trained to help resolve these issues.

Often families don’t recognize the need for assistance until faced with a crisis. To make it easier for families and loved ones, we encourage you to look ahead, and work with us to plan for the future needs of your aging loved ones.

Our Rates

Care Management/per hour - $115

All care management services are billed on a monthly basis. Payment is due upon receipt of statements.
